How to Add a New Form Version
The Multiple Form Versions feature allows you to create and manage several form designs at once. This makes it easy to experiment, optimize, and switch between versions without losing previous configurations. You can also tie different products or collections to different form versions, ensuring that each item in your store displays the form that best fits it.
NOTE: This feature is only available on the new version of the app.
Step 1: Access the Form Versions
- Open the app and go to the Form Designer section.
- In the top-right corner you will see a list of all your available versions, including the Default form.
Step 2: Create a New Version
- To create a new form click on New Version.
- In the setup window you can :
- Name your new form version.
- (Optional) Copy version from an existing one to duplicate settings.
- Select visibility rules for Products: All products, Specific products or None
- Select visibility rules for Collections: All collections, Specific collections or None
Step 3: Personalize your New Form as you like.
Each form version can be:
- Edited – Change its fields, layout, and settings.
- Renamed – Update the version name.
- Deleted – Remove it (note: the Default version cannot be deleted).
- Set as Default Form – IMPORTANT: When you do this, the chosen version, for example one called “test,” will be promoted to become the default form. This means it will always be shown when no other version rules match. The current default will not be lost. Instead, it will automatically be renamed and saved as a regular version with the name “test (Old),” keeping the same visibility rules it had before. The new default will have no restrictions and act as the universal fallback. All form content, styling, and fields from both versions will be fully preserved.
Step 4: Save and Test
After creating or editing a version, click Save to apply your changes. Then, visit your store to check how the correct version displays based on the rules you set. If necessary, return to the Form Designer to make adjustments.

Yes, you can create as many versions as you need. Each version can be customized independently.
